GST (Goods and Services Tax) has been in effect in India since July 2017, but many clinic doctors still have questions about when and how it applies to their practice. This guide covers the key points every small clinic owner should understand about GST billing in India.

Are medical services exempt from GST?

Yes — healthcare services provided by a clinical establishment, an authorised medical practitioner, or paramedics are exempt from GST under Notification No. 12/2017 — Central Tax (Rate), Entry 74.

This means:

  • Consultation fees charged by a doctor are not subject to GST
  • OPD charges are GST-exempt
  • Surgical procedures performed at a clinic are GST-exempt
  • Physiotherapy, diagnostic services, and nursing care provided as part of healthcare are GST-exempt

In practical terms: most small clinic doctors do not need to charge GST on their consultation or procedure fees.

When does GST apply to a clinic?

While core medical services are exempt, GST may apply to certain activities at a clinic:

  • Cosmetic procedures: Procedures that are primarily cosmetic and not medically necessary (e.g., cosmetic surgery, Botox purely for aesthetics) may attract 18% GST.
  • Optical retail: Sale of spectacles, contact lenses, and frames at an optical shop — these are goods and attract GST (typically 12% on frames, 12% on lenses).
  • Pharmacy sales: Medicines sold at a clinic pharmacy attract GST at the applicable pharmaceutical rate (5% or 12% depending on the drug).
  • Renting out clinic space: If you sublet a room in your clinic to another doctor or entity, this may be taxable under GST as a service.

For most general practice, dental, eye, ENT, physio, and ortho clinics focused purely on clinical services — GST does not apply to the core consultation and treatment fees.

Does a clinic need to register for GST?

GST registration is mandatory only if your aggregate annual turnover exceeds ₹20 lakhs (₹10 lakhs in special category states). However, since core healthcare services are exempt, this threshold is counted only against taxable services, not exempt ones.

For most small clinics earning primarily from consultations and exempt procedures:

  • If all your revenue is from exempt healthcare services, GST registration is not required regardless of turnover.
  • If you have some taxable supplies (e.g., pharmacy sales above the threshold), you may need to register.

What billing records should a clinic maintain?

Even if you're GST-exempt, it's good practice to maintain proper billing records for:

  • Income tax filing (clinic revenue counts as professional income)

  • Patient-facing receipts (patients increasingly ask for official receipts for insurance reimbursement)

A good clinic billing practice includes:

  • Recording every patient payment with date, amount, payment method (cash/UPI), and service description
  • Tracking clinic expenses separately (rent, supplies, salaries) from patient revenue
